All faculty members must submit an annual report and an updated CV to their department chair and the Vice President for Academic Affairs/Provost by the first class day of spring semester. The report should include the following information:

I. Statement of current responsibilities in the areas of teaching, service, and scholarship

II. List of achievements in the following areas

A. Teaching and Advising
B. Service
C. Scholarship (Please include the name of any endowed Chair, Professorship, or Support Funds that have supported scholarship.)

III. Reflections on Selected Work

A. Goal of work
B. Preparation

1. Workshops and/or special training
2. Research by reading, study and investigation in the field
3. Acquisition of new skills necessary for project

C. Describe the methods used in your project to achieve goal(s).

D. Communication of work to others.

E. Expected impact of the work on the larger academic world.

F. Methods used to critically evaluate the work. Impact the evaluation will have on future work.

IV. New plans

A. Goals for the coming academic year

B. Suggestions for the department and the College.
