Self-Identifying and Requesting Accommodations

Academic Accommodations can be requested by students enrolled at Centenary College of Louisiana. Students seeking accommodations are required to self-identify their disability and need for accommodations with the Disability Services office. Disability Services will not seek out students with disabilities. Students may self-identify at any point during the semester. Students will fill out the Disability Services Request Form to initiate the process.

How to Register

To be considered for accommodations, students will be asked to provide documentation that supports a connection between disability and the requested accommodation(s). Centenary’s documentation guidelines [link PDF here] are guided by the recommendations of the Association on Higher Education and Disability (AHEAD). Documentation from external sources should come from health care providers, mental health providers, school psychologists, or other appropriate professionals.

We accept any of the following:

  • Individual Education Program (IEP) or 504 Plans (must include diagnostic information)
  • Psychoeducational/Neuropsychological/Cognitive Evaluations
  • Medical Records
  • Educational Records
  • A copy of approved accommodations for standardized examinations (ACT, SAT, LEAP, etc.)
  • Disability Certification Form completed by a licensed medical professional with knowledge of the student.

The required documentation may vary depending on the nature of a student’s disability. Its relevance and value are dependent on the original context, credentials of the evaluator, the level of detail provided, and the comprehensiveness of the narrative. Disability Servicescannot proceed with the intake process unless appropriate supporting documentation is provided. During the review process, Disability Services may request additional information.

Each student will be evaluated individually through a review of documentation and an interactive meeting. We recognize the value of the student’s experience and perspective and therefore work collaboratively to develop an accommodation plan together. Accommodations are not one-size-fits-all. Documentation of the disability is required from students seeking accommodations. It is a vital part of the accommodation process which equips Disability Services with the ability to make an informed decision regarding the need for the accommodations and the type of accommodations that may best serve the student.

Accommodation Notice

Upon completion of the review process, the Disability Services office will notify relevant college staff (faculty, campus offices, etc.) via email. The student is copied accommodations notices via email. Upon receipt of the email notification, the student should schedule a meeting with their faculty to discuss the specific accommodations outlined and how they will be implemented. It is in the best interest of the student to do this early in the semester, as accommodations are not retroactive.
